It depends on who the bus operator you're using.
Those that I took, will stop at Yong Peng, the first stop. The second will be within KL area. Could be just before or after KL. The third stop is along the expressway, in between KL and Tapah. Sorry, not sure the name of it for most of the time I was too groggy to bother finding out the name.
For urgent cases, don't be afraid to ask the driver. No matter how foul his moods may be. He have two options. Stop and let you relief yourself or. Continue driving and you pee/shit in his bus. A person with good sound mind will know which option to take.
But do remember to thank him after he fulfill your request. 
On a side note, I don't drink any fluids 1.5hrs before the bus departs. I will try to relieve myself at the Singapore Customs or at the Malaysian Customs. During the whole trip, I will only take small gulps just to wet the mouth. I will never consume anything more than that. This will reduce the need to make too many visit. Everytime the bus stop, I will make an effort to relieve myself.
